Tumor Necrosis Factor alpha (TNF alpha) is a versatile proinflammatory cytokine belonging to the TNF superfamily. Its biologically active form is a homotrimer with a molecular mass of 52 kDa, while the monomeric form remains inactive. TNF alpha is produced as a transmembrane protein in humans and assembled intracellularly into a noncovalently linked homotrimer before being expressed on the cell surface. It is predominantly secreted by macrophages and monocytes during acute inflammation and plays a critical role in cellular signaling pathways that result in necrosis or apoptosis. TNF alpha is also essential for immune defense against infections and cancers.

The effects of TNF alpha are mediated through binding to TNFR-1 or TNFR-2. Both receptors exhibit similar affinity for TNF alpha and can activate NF-κB. However, TNFR-1 uniquely contains a cytoplasmic death domain that triggers apoptosis. Soluble forms of both receptors are present in human serum and urine, neutralizing TNF alpha’s biological activity.
Physiologically, TNF alpha is crucial for regulating immune responses. However, its overproduction or dysregulation can contribute to various diseases, including asthma, Crohn’s disease, rheumatoid arthritis, neuropathic pain, obesity, type 2 diabetes, septic shock, autoimmunity, and cancer. Its central role in chronic inflammatory and autoimmune diseases highlights its importance in both research and clinical applications.
